The following information needs to be completed by all volunteers ministering to minors – the requirements by the Archdiocese can be accessed below – ALL five (5) sections need to be completed for the safety of the children of St. John of the Cross.

* 1. Archdiocese Background Check:

Print out the details to complete the Application On-line Instructions
Log on:  Background Check -  complete all sections online

* 2. Virtus – PGC Workshop:

Select a convenient workshop at the link Protecting God’s Children Workshop and register to attend. If workshop is not at St. John of the Cross, please submit a copy of the Certificate of Completion to the YC Office

* 3. Code of Conduct:   Read, print only page 6, sign and return to YC office -  Code of Conduct form

*4. CANTS:  Print, read, sign and return to YC office (DO NOT mail to DCFS) - CANTS form

*5. Mandated Reporter Training: 

Read details to complete the online training for Mandated Reporter 
Log on:  Madated Reporter Training Online
